Healthy Skin Portal utilizes a variety of automated contact forms that enable guests to request information and services. Through these contact forms we may collect:

• Information regarding guests' skin treatment interests

This information enables us to process and respond to guest service inquiries in a prompt and accurate manner.

Contests / Surveys / Quizzes

We may occasionally run contests, conduct surveys, or feature quizzes in which we ask visitors for contact information (such as email addresses), personal feedback (such as procedure interests) and unique identifiers (such as member numbers). The information collected on our contest applications is used to administer the contest and to notify winners.

The information collected in surveys is used to improve our website's relevancy, accuracy and service capabilities. Quiz information is used to personalize guests' experiences and treatment inquiries through our site. In the event a guest doesn't want to be contacted following a survey or quiz, guests may choose to opt out of future contact correspondence with Healthy Skin Portal.

Financial Information

Guest financial information is never collected directly through the Healthy Skin Portal website. If a guest has contacted Healthy Skin Portal to schedule a skin treatment consultation, and if the preferred skin treatment specialist charges a consultation fee, a cancellation fee or a no-show fee, guests will be asked to submit a credit card or billing account number to a Healthy Skin Portal liaison representative over the phone to hold their appointment. In cases where the consultation appointment has no attached fees, guests won't be required to submit any financial information to us.

If we must collect a guest's credit card information, it is always stored in Healthy Skin Portal's secure database and is only viewable by select administrative staff.

Demographic Information

Demographic guest data may also be collected through Healthy Skin Portal. We use this data to personalize the guest's experience with us, and to show them content that we think will be relevant or interesting to them. For guests who become members of Healthy Skin Portal or Signature Forum (Healthy Skin Portal's parent company site), this information will help us provide them with content according to their preferences.

Additionally, this information may be used internally by Healthy Skin Portal for market analysis and other purposes. In select circumstances, this information may be shared with advertisers on an aggregate basis (i.e. all personal information will be removed).

Mailing Lists

Contact information registration forms, contest forms, survey forms and other sources are also used to send information about us, and promotional materials, to our guests.

If and when you place orders* through our website, or through our parent site Signature Forum, you will transmit your order information, including your credit card number, to our secure server. Our secure server uses Secure Sockets Layer Software (SSL) which allows us to receive information that your web browser encrypts before transmitting to us.

Since any order information you provide to us will be transmitted using a secure connection, customers with web browsers that cannot support this level of security will not be able to order products through Healthy Skin Portal or Signature Forum.

To ensure that you are conducting a secure transaction, please look for a small padlock icon in the lower right corner of your browser before you send your financial information. Should this icon not be available, please notify Healthy Skin Portal immediately and DO NOT send your financial information.

The security of an online transaction* is of the utmost importance to our clients and their customers. This exchange is secured using Secure Sockets Layer technology from Netscape. SSL is considered the standard in the industry and has been adopted by web software developers. SSL is used to receive your order, encrypt it and, through authentication and data encryption, secure all aspects of the online transaction.

Healthy Skin Portal does not sell or share guest information with any unaffiliated third parties. Healthy Skin Portal will share guest information with select network doctors and their office staff in the event that a guest schedules an appointment with one of our doctors.

If Healthy Skin Portal must acquire and share guest financial information with one of our doctors to schedule an appointment, this information won't be relayed via insecure emails or faxes. We communicate required guest financial info directly to the doctor or their office manager over the phone.

Account Information

Healthy Skin Portal may release account information if it believes, in good faith, that such release is reasonably necessary to (1) comply with the law, (2) enforce or apply the provisions of our Terms and Conditions, or (3) protect the rights, property or safety of Healthy Skin Portal, our customers or other individuals.

Credit Verification

Please note that when you order or schedule services (like consultations) from Healthy Skin Portal, we will use a secure credit verification company to validate the credit card and other information you provide, at the request of the plastic surgeon, and before we fulfill your order. A $1 validation charge will appear on your card statement following this process and will be credited back to you shortly thereafter. We have an agreement with said credit company to ensure that guests' personal information is kept confidential.

"Cookies" are pieces of information that our website can send to your browser and which can later be stored on your computer's hard drive so that we can recognize you when you return. Any pages on the Healthy Skin Portal web site where you are prompted to log in or that are customizable require that you accept cookies.

Our cookies do not contain any personally identifying information, but they do enable us to recognize you when you return to our site, and/or save unfinished activities between visits. Most web browsers automatically accept cookies, but you can usually change your browser to prevent that. Even with cookies disabled, you can still use most of the features on our website.
